Discworld naming conventions

I have taken the liberty to move Discworld to Discworld (world), following the example at Wikipedia:Wikipedia:WikiProject Discworld:

• When talking about the books it is Discworld (''[[Discworld]]'').
• When talking about the world it is the Discworld (the [[Discworld (world)|Discworld]]).

I also believe that:

• All book titles should be italicised and, in the first instance, linked.
